Nurse Practitioner NP - Physician Assistant PA - Wound Ostomy - Clackamas
Provide high-quality health care through active participation in care for patients in Wound Ostomy Department via direct assessment and formulation/implementation of treatment plan. Performs complete work-up on patients, including H & P, ordering of diagnostic tests, coordination of care with multi-disciplinary healthcare team, and patient teaching. Document in writing all care in patients permanent medical record in accordance with accepted medical/legal standards. Provides all medical care in accordance with accepted practice standards, hospital/clinic protocols (including supervision by a physician), and State licensing guidelines.
- Provide direct assessment, diagnosis, and formulation/ implementation of treatment plan for patients presenting with acute and/or chronic wounds in the ambulatory care setting. Outpatient responsibilities include histories and physicals, exams, ordering diagnostic studies and medications, screening referrals, and wound management. Significant time is spent handling patient inquiries regarding care plan and coordination of care with members of multi-disciplinary healthcare team.
- Venous and lymphatic disease: provide direct assessment, diagnosis, and formulation/ implementation of treatment plan for patients presenting with venous stasis and/or lymphatic disease. Outpatient responsibilities include histories and physicals, ordering diagnostic studies and medications, application of the principles of compression therapy, screening referrals. Significant time is spent handling patient inquiries regarding plan of care.
- Neuropathic ulcers including Diabetic Foot Ulcers (DFUs): provide direct assessment, diagnosis, and formulation/ implementation of treatment plan for patients presenting with neuropathic ulcers. Outpatient responsibilities include histories and physicals, ordering diagnostic studies and medications, screening referrals. Significant time is spent handling patient inquiries regarding care plan and coordination of care with multi-disciplinary healthcare team.
- Review and manage imaging results including X-rays, US, CT scans, MRA, angiograms as relates to wound healing and in coordination with multi-disciplinary healthcare team. Duties include ordering future appropriate diagnostic studies and care management of wounds. Significant time is spent with care coordination with patient and healthcare team.
- Minimum 1 (one) year of experience managing complex wounds.
- Graduate of approved PA or NP program with certification.
- Bachelors degree as PA, NP, or closely related field.
- This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.
- Nurse Practitioner License (Washington) within 6 months of hire AND Nurse Practitioner National Certificate required at hire AND Registered Nurse License (Oregon) within 6 months of hire AND Nurse Practitioner License (Oregon) within 6 months of hire AND Registered Nurse License (Washington) within 6 months of hire
OR
- Physician Assistant License (Washington) within 6 months of hire AND Physician Assistant National Certificate required at hire AND Physician Assistant License (Oregon) within 6 months of hire
- National Provider Identifier required at hire
- Basic Life Support
- Advanced Cardiac Life Support
- Drug Enforcement Administration Registration
- Thorough and working knowledge of wound care and wound assessment and management, especially in diabetic feet, and chronic woundsWorking knowledge of vascular diseases and venous diseases and their treatment.
- Ability to use appropriate procedural/testing equipment, to see large volume of patients, and to interface with large
- provider and support staff.
- Masters degree as PA, NP, or closely related field.
- Graduate of ANA-approved nurse practitioner training program.
- Additional training in wound care.
- Compact License is acceptable in lieu of a Washington RN license.
