Job Summary:

The Renal Care Manager Team Lead (RCM TL) assures delivery of high quality, cost-effective care to Kaiser Permanente Northwest Nephrology patients by acting under the direction and in partnership with Management to provide clinical leadership for the MDT (Multi-Disciplinary Team) through triage, utilization management, compliance/quality review, and direct and indirect patient care.

Essential Responsibilities:

  • The Renal Care Manager Team Lead (RCM TL) will assure delivery of high quality, cost-effective care to Regional Nephrology patients by:
  • In collaboration with Nephrology Department Manager or Director, determines the staffing and scheduling needs for their designated area of responsibility. Acts in a cost-conscious manner to recommend canceling, teaming and mix alterations as fluctuations in demand occur.
  • Triaging or assigning triage of CKD, DPC, ESRD, and Txp referrals (Chronic Kidney Disease, Dialysis Placement Coordinator, End Stage Renal Disease, and Transplant referrals), on same business day of entry, no later than following business day.
  • Providing or assigning provision of authorization/reauthorization for all necessary referrals.
  • Providing or assigning provision of utilization management for internal and external Nephrology-related services.
  • Works in partnership with Nephrology Manager or Director and Nephrology physician leadership to facilitate interdisciplinary team functioning.
  • Assisting and participating in the hiring of Nephrology MDT (Multi-Disciplinary Team) employees. Develops and maintains orientation checklist. Provides orientation and precepting for Nephrology MDT employees, as applicable.
  • Provides input to the Manager or Director for development of Performance Appraisals of Care Managers, Social Worker, Dialysis Placement Coordinators, and unlicensed personnel in the Department or service.
  • Assisting with pro-active coaching of staff performance, providing input to staff evaluations, and identifying and reporting problems that impact delivery of patient services.
  • Acting as a clinical resource to inter/intra-department staff.
  • Assessing staff educational needs within the Department and facilitate the delivery of appropriate in-service experiences.
  • Providing ongoing education and mentoring to regional staff and providers on changes to policies and guidelines.
  • Maintaining and updating triage agreements and communicating changes to PAS and region.
  • Collaborate with MDT to identify stakeholders to work with Nephrology Management by participating in appropriate policy/procedure development at meetings, in equipment purchase decisions, and maintenance plans.
  • Assuring inter-department coordination between the Nephrology group and other groups within the Region by acting as a liaison to other departments within Kaiser Permanente and outside agencies.
  • Leads case conference/huddles as appropriate.
  • Triaging regional patient referrals, managing access, wait lists, and referral processes in partnership with NWP Nephrology leadership and Department Manager.
  • Nursing Practice: Provide and document quality patient care and services utilizing the nursing process consistent with the plan of care directed by the Nephrologists.
  • Nursing Practice: Performs or appropriately delegates independent nursing functions as defined in the Oregon and Washington Nurse Practice Acts.
  • Nursing Practice: Communicates patient needs and changes in condition to appropriate personnel.
  • Nursing Practice: Functions as a patient advocate within the scope of a Health Maintenance Organization.
  • Nursing Practice: Maintains a safe and therapeutic environment for staff, patients and family members.
  • Nursing Practice: Uses cost effective measures in the delivery of nursing care.
  • Nursing Practice: Coordinates and implements medical orders.
  • Nursing Practice: Performs skilled nursing tasks and demonstrates operational knowledge of procedures and equipment.
  • Nursing Practice: Performs triage and telephone advice independently and within scope of the Oregon and Washington Nurse Practice Act.
  • Nursing Practice: Coordinates activities of the interdisciplinary team to ensure positive outcomes for individual patients with complex health care needs.
  • Nursing Practice: Develops or delegates development of nursing care plans for high-risk patients, and populations, cared for within Nephrology, as appropriate.
  • Nursing Practice: Identifies patient/caregiver knowledge deficits associated with health care and/or the therapeutic regimen and responds appropriately.
  • Exhibits professional behavior supporting regional and departmental policies, procedures goals and values. Activities include performing or assigning initial and ongoing patient assessments, functional needs and abilities, compliance with physician ordered care, psychosocial status, need for community resources and rehabilitation services.
  • Actively seeks opportunity for continued professional growth and knowledge.
  • Coordinates the delivery of Nephrology care with nursing and social work to Kaiser Permanente and non-Kaiser Permanente providers which results in effective utilization of resources and the timely attainment of achievable outcomes.
  • Guides members of MDT, as applicable, through on-site evaluation of the provision of care, conferencing and review of their documentation according to State Practice Act and agency policy/procedure.
  • Actively participates in compliance and quality management and improvement activities within the region including but not limited to
  • Clinical record review
  • Occurrence reporting
  • Process and outcome measurements
  • Peer review, and
  • Continuous improvement projects in collaboration with the UBT co-lead(s), as appropriate.
  • Assists supervisors/managers in the resolution of KP Member complaints and compliance/quality concerns.  
  • Coordinates intra-office emergency systems (fire drills, codes) with other Team Leaders. Collaborates with other Team Leaders to effectively and efficiently utilize resources in delivering quality care to members.
  • Performs other duties as requested.
Basic Qualifications:

Experience


  • Minimum two (2) years leadership (Team Lead, Preceptor, Supervisor, or Manager) experience or commensurate experience, prefer some budget development.

  • Minimum five (5) years dedicated Nephrology nursing experience.


Education


  • Bachelors degree in nursing and four (4) years of experience in a directly related field working with renal patients.

  • High School Diploma or General Education Development (GED) required.


License, Certification, Registration


  • Dialysis Nurse Certificate required at hire OR Nephrology Nurse Certificate required at hire



  • Registered Nurse License (Oregon) required at hire



  • Registered Nurse License (Washington) required at hire



  • Basic Life Support required at hire



Additional Requirements:


  • Working knowledge of the Renal managed care industry.

  • Knowledge of CMS regulations/requirements.

  • Demonstrates competency Renal modalities.

  • Demonstrates effective interpersonal communication.

  • Able to prioritize patient needs to achieve attainable treatment goals.

  • Thorough knowledge of teaching/adult learning principles.

  • Working knowledge of interdisciplinary care coordination.



Preferred Qualifications:


  • Excellent time management, problem solving, and customer service skills.

  • Experience as an interdisciplinary team member.

  • Excellent written and verbal communication skills.

  • Able to work autonomously and manage their caseload.
