Pharmacy Care Delivery Specialist VI, Home Delivery Services
In addition to the responsibilities listed below, this position is also responsible for facilitating work within centralized services including mail order, fulfillment, prescription processing and/or contact center functions; mentoring others to ensure performance standards are met in pharmaceutical care, service, and operations across outpatient pharmacies; providing recommendations and leading implementation of regional and national strategies and initiatives to improve member experience and promote home delivery adoption in accordance with business goals; providing guidance on matters regarding program development, implementation, and communication flow with the Pharmacy Clinical division and with outside departments; ensuring proper integration of centralized support services and technology into pharmacy operations and workflows; addressing centralized pharmacy space needs; using innovative logic to drive state-of the-art solutions to determining gaps and ensure the operation of the departments equipment, including the provision of preventative maintenance, and all other engineering activities relates to the equipment and technology used in the department.

- Drives the execution of multiple work streams by identifying member and operational needs; translates business strategy into actionable business requirements; develops and updates new procedures and policies. Gains cross-functional support for objectives and priorities; determines and carries out processes and methodologies; solves highly complex issues; escalates and resolves issues as appropriate; sets standards and measures progress. Develops work plans to meet business priorities and deadlines; coordinates, obtains and distributes resources. Removes obstacles that impact performance; guides performance and develops contingency plans accordingly; influences the completion of project tasks by others.
- Leads drug education and training efforts by: developing highly complex trainings to continue drug education and staff development in-services for pharmacists, physicians, and other health care professionals, and students.
- Provides pharmaceutical patient care and drug therapy by: fostering productive relationships with care providers and members; providing highly complex case-specific drug information (e.g., drug therapy, adverse effects, compliance, appropriate use, and handling) to health care providers and members, independently; researching, organizing, identifying complex issues, and/pr recommending patient-specific drug therapy plans while evaluating progress of therapy through interviews, physical assessment, patient education and clinical laboratory monitoring, independently; analyzing, identifying trends, and presenting highly complex therapeutic outcomes to senior health care providers to collaboratively identify state-of-the-art drug plan improvements, independently; integrating implementations, evaluating, and reporting utilization, as appropriate, of targeted medications and medication classes to senior leader, strategizing novel work to assure safe, rational and cost-effective prescribing; guiding the development and implementation process while also analyzing clinicians or department-level decisions, support, and feedback using tools to facilitate engagement and support for the drug use management process; and leading the coordination of highly complex pharmaceutical care and optimal utilization of resources to and from acute and ambulatory patient-care settings.
- Leverages, maintains, and complies with all existing internal and external quality, safety, emergency, and accreditation policy and procedures by: adhering to all regulatory rules and regulations (e.g., Drug Enforcement Administration [DEA], State Board of Pharmacy, Health Insurance Portability and Accountability Act [HIPAA]) and relevant internal regional and national policies; ensuring patient safety in the highly complex preparation and provisioning of care (e.g., medications, procedures, infection prevention), ensuring continued optimization of patient safety and success; implementing process improvements to prevent safety hazards, accidents and incidents, and unsafe working conditions promptly; and collaborating with physicians, other medical professionals, and health plan personnel to anticipate potential issues, proactively resolve highly complex, and assure quality and process outcome measures.

- Doctorate of Pharmacy or equivalent degree (e.g., Bachelors of Pharmacy from a School of Pharmacy) AND completion of a Post-Graduate Year 1 (PGY1) residency at time of hire AND minimum four (4) years of experience as a Pharmacist and/or Pharmacist Intern in pharmacy or a directly related field OR Doctorate of Pharmacy or equivalent degree (e.g., Bachelors of Pharmacy from a School of Pharmacy) AND minimum six (6) years of experience as a Pharmacist and/or Pharmacist Intern in pharmacy or a directly related field.
- Minimum five (5) years of experience in a leadership role with or without direct reports.
- Pharmacist License (California) required at hire
- Two (2) years of experience utilizing automation tools and technologies to streamline processes, improve efficiency, and enhance productivity in high volume pharmacies.
- Two (2) years of experience consulting in a strategic capacity on highly complex or highly visible projects, programs, or initiatives.
- Four (4) years of experience designing effective health communication strategies or products.
