Job Summary:

To process electronically routed (scanned / e-faxed) health information documents for inclusion in the electronic medical record within the scanning solution environment.  This includes, but is not limited to, accurate member association, indexing to appropriate section by document type and other descriptors, attaching to orders and encounters, validating resolution quality, and identification of routing into the scanning system front-end application server (VEC) prior to routing to the IDM final document state that will send to KP HealthConnect Clinicals module. Individuals in this position will be required to meet specified strict deadlines with a high volume, high accuracy rate on a daily basis.

Essential Responsibilities:


  • Indexes health information documents and insures accurate associate on member, section, encounter, and order levels.  This includes reviewing document type master index to identify type of document and determine whether or not document is appropriate to be incorporated in patient health record.

  • Suspends documents without identifiable document type or membership issues for quality review.

  • Systematically commits documents for incorporation in the patient health record using vendor software.

  • Reviews documents to insure appropriate frame, resolution, orientation, etc. and adjusts accordingly or notifies appropriate staff if rescanning is required.

  • Manual review of each scanning batch manifest log assuring that total pages listed on batch manifest log matches total pages in batch, retrieves as appropriate.

  • Problem solves routing disparity issues related to scanned batches and document type issues.

  • Able to identify originals of scanned images/documents as necessary, when rescanning is required.

  • May be required to answer phones in order to meet customer service standards.

  • Participates in resolution of unknown document types and department driven descriptions as assigned.

  • Maintains specified timeliness, quality and production standards.
Basic Qualifications:

  • Minimum one (1) year work experience in record handling in a medical setting required.

  • High School diploma or GED required.
License, Certification, Registration

Additional Requirements:

  • Working knowledge of medical terminology.
  • Working knowledge of PCs and Windows based environment. Systems to be used will include KP HealthConnect Clinicals and scanning vendor software.
  • Proficient in member identification research in common membership (IRCS).
  • 70% overall score of Word Essentials required.
  • 70% overall score of Medical Terminology required.
  • Minimum of 6132 keystrokes per hour for Alphanumeric data entry required.
Preferred Qualifications:

  • Experience in health care and/or managed care electronic environment in health information / medical records preferred.
  • Knowledge of health information management.
  • Working knowledge of Windows, Lotus Notes, and Microsoft Word.
  • Knowledge of Epic Inbasket.
  • Must be extremely detail oriented.
  • Able to function in a fast-paced environment.
