- Upholds Kaiser Permanentes Policies & Procedures, Principles of Responsibility, and applicable state, federal and local laws.
- Functions within the scope of the Nurse Practice Act in the state(s) for which they are licensed and in accordance with the American Nurses Association Scope and Standards
- Utilizes the Nursing Process:
- Assessment: conducts thorough and systematic assessments of patients physical, psychological, social, and spiritual needs; gathers pertinent data through observation, interviews, and diagnostic tests; and utilizes critical thinking skills to analyze assessment findings and identify actual and potential health issues.
- Diagnosis: formulates nursing diagnoses based on comprehensive assessment data and recognized nursing standards; collaborates with healthcare team members to identify actual and potential health problems; and prioritizes nursing diagnoses to address the most urgent patient needs.
- Planning: develops individualized care plans in collaboration with patients, families, and healthcare team members; establishes measurable goals and outcomes that are realistic and achievable within a specified timeframe; and incorporate evidence-based practice guidelines and nursing interventions into care plans.
- Implementation: provides skilled nursing care based on the established care plan and nursing interventions; educates patients and families on disease processes, treatment options, and self-care strategies.
- Evaluation: continuously assesses the effectiveness of nursing interventions in achieving desired patient outcomes; modifies care plans as needed based on ongoing evaluation and reassessment of patient responses; and documents all aspects of patient care, including interventions, responses, and changes in condition.
- Promotes Patient Centric Care:
- Patient Advocacy: serve as an advocate for patients and families, ensuring their voices are heard and their rights are respected; empower patients to actively participate in their healthcare decisions and treatment plans; addresses concerns, complaints, and feedback with empathy and professionalism.
- Individualized Culturally Appropriate Care: collaborates with patients, families, and healthcare team members to develop personalized care plans; collaborates with social workers, chaplains, and other support services to address patients psychosocial needs; identifies patients unique needs, preferences, and goals to inform the development of tailored care strategies; uses knowledge of cultural differences to provide nursing care that is sensitive to and consistent with cultural values, beliefs, and customs.
- Communication and Education: foster open and honest communication with patients and their families, providing clear explanations of diagnoses, treatments, and procedures; educates patients about their health conditions and self-care techniques to promote empowerment and independence; utilizes patient-centered communication techniques to build trust and rapport with individuals from diverse cultural and socioeconomic backgrounds.
- Collaborates within Care Teams:
- Interdisciplinary Collaboration: collaborates closely with healthcare professionals to develop and implement integrated care plans; participates in interdisciplinary team meetings to discuss patient progress, treatment goals, and care coordination strategies.
- Interdisciplinary Communication: facilitates clear and open communication among healthcare team members, ensures care team is informed and aligned regarding patient care plans and objectives; relays important patient information, updates, and changes in condition to relevant team members in a timely manner.
- Viewed by others as an effective team member who is flexible, cooperative, and willing to assist others acting as a resource to other team members.
- Confronts difficult or conflict situations constructively and seeks appropriate assistance when necessary.
- Technology Enabled Care/Informatics:
- Electronic Health Records (EHR) Management: navigate and utilize EHR systems effectively to document patient assessments, interventions, and outcomes accurately and efficiently; analyzes electronic health data to inform clinical decision-making, care planning, and quality improvement initiatives; and complies with documentation standards, privacy regulations, and security protocols.
- Telehealth and Telemedicine Support: facilitates virtual patient consultations and follow-ups using telehealth platforms, video conferencing tools, and remote monitoring devices; educates patients and families on how to access and use telemedicine services for remote healthcare delivery and monitoring.
- Technology Integration and Innovation identifies opportunities to integrate technology into nursing practice to enhance efficiency, communication, and patient engagement; participates in the evaluation, selection, and implementation of new technology solutions to meet the evolving needs of patients and healthcare providers.
- Leadership and professionalism:
- Takes accountability for ones own actions and accepts constructive feedback
- Serves as a leader of the healthcare team; prioritizing, delegating, and assigning tasks appropriately to ensure completion of patient care activities optimal patient outcomes
- Understands other team members scope of practice/scope of duties for appropriate assignments and escalations when necessary
- Participates in problem identification and resolution including problem solving with other departments to assist with resolution to meet member needs.
- More than 1 year of RN experience in any practice setting.
- Associates degree
- This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.
- Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire
OR
- Compact License: Registered Nurse within 6 months of hire
- Registered Nurse License (District of Columbia) within 6 months of hire
- Basic Life Support from American Heart Association
- Successful completion of the selection process (i.e. pre-interview shadow experience).
- Current Kaiser Permanente RN preferred.
- Bachelors degree in nursing or related field.
- Must have BLS certification and obtain PALS, ACLS and ENPC certifications.
